Do amalgam fillings cause tooth loss
The short version: amalgam fillings release mercury into the body, but the evidence connecting that exposure to tooth loss specifically is thin and indirect.
The evidence you provided shows that amalgam fillings do release measurable mercury. Women with long-term amalgam fillings had higher mercury, aluminum, silver, and barium in their hair, along with signs of oxidative stress [1]. Children’s urinary mercury rose significantly after getting amalgam fillings [5]. Mercury from maternal fillings crosses the placenta and appears in breast milk [2, 11, 18]. One study even found that a 7-Tesla MRI caused a large mercury spike from amalgam in a lab dish [3], though a later study with 1.5-T and 3-T MRI found no significant release [24].
But does this mercury exposure cause teeth to fall out? None of the studies you provided directly test that question. The closest evidence is about how well amalgam fillings hold up mechanically. A large retrospective study of nearly 11,000 restorations found that cusp fracture rates were about the same for amalgam (1.88%) and resin composite (2.29%) [25]. A 5-year randomized trial in Brazil found that amalgam and resin composite restorations had similar survival rates (83% vs 75%) [32]. A pilot study over 5 to 10 years found that tunnel restorations failed more often than minimal amalgam restorations [33]. These studies suggest amalgam is a durable material, not that it causes tooth loss.
The evidence on systemic harm from amalgam mercury is mixed. A randomized trial in children found no significant difference in IQ or kidney function between those who got amalgam fillings and those who got composite fillings over five years [20]. A twin study in older adults found no link between amalgam burden and physical or mental health [9]. But a 2025 review of mercury and neurodegenerative disease concluded that mercury is neurotoxic, even if population studies do not consistently link it to Alzheimer’s or Parkinson’s [13]. A study from 1990 found that people with amalgams had higher blood pressure and lower hemoglobin than those without [17].
The evidence does not answer whether amalgam fillings cause tooth loss. The mechanical studies show amalgam is a durable filling material. The toxicology studies show mercury exposure is real and measurable. But no study in this set tracked whether people with amalgam fillings lose more teeth over time than people without them. That gap matters.
