The short version: a 7-day course of amoxicillin-clavulanate is effective for bacterial sinusitis, but the safety data shows real risks you should weigh.

The retrieved evidence shows this antibiotic works. In adults with acute bacterial sinusitis, a 7-day course of amoxicillin-clavulanate 875/125 mg produced significant symptom improvement, similar to inhaled alternatives [8]. A 2006 manufacturer-funded study of 1,678 patients reported 87.8% bacteriologic success, with diarrhea in 16.5% and 2.2% withdrawing due to side effects [9, 10].

For children, a 10-day course was similar in efficacy to a 3-day azithromycin course, with no severe adverse reactions attributed to either [5]. In acute otitis media studies, amoxicillin-clavulanate caused more gastrointestinal side effects (39% vs 18%) and diaper rash than alternatives [3, 7].

The serious side effects are rare but real. Italian reporting data found amoxicillin-clavulanate caused more liver damage (4% of reports) and Stevens-Johnson syndrome than amoxicillin alone [1]. A 2024 case report describes severe toxic epidermal necrolysis after the drug [2], and another describes severe cholestatic liver injury in an 80-year-old woman [4]. These are documented, not hypothetical.

The evidence does not include a direct safety comparison of a 7-day course against longer or shorter courses for sinusitis specifically. Most sinusitis trials used 10-day courses. A 5-day regimen was shown non-inferior to 7 days for chronic bronchitis [6], but that is a different disease.

For a typical patient, the most common side effects are diarrhea (about 1 in 6), nausea, and skin rash. Serious liver injury or severe skin reactions are rare but possible. The 7-day duration is standard and supported, but not rigorously safety-tested against shorter courses for sinusitis.

My call: amoxicillin-clavulanate is effective for bacterial sinusitis but carries real, documented risks including diarrhea, liver injury, and severe skin reactions. In children, alternatives cause fewer side effects. Confidence: moderate for efficacy; moderate for safety risks being real, though low for the exact 7-day sinusitis-specific incidence.
