The short version: no studies examine COVID-19 vaccine safety in former IV opioid users. The general-population evidence on myocarditis and clots is real but small, and the retrieval cannot see the mechanisms that matter most for this group.
The 43 records above contain zero studies that test COVID-19 vaccine safety in people with a history of intravenous drug use. That population has distinct vulnerabilities: higher rates of hepatitis C, HIV, endocarditis, and chronic immune activation from repeated infections and adulterants. None of the retrieved studies examine this group. The question cannot be answered from direct evidence.
What the records do show about myocarditis and clots in the general population is a consistent signal. mRNA vaccine myocarditis is concentrated in young males after the second dose, with rates around 12.6 cases per million doses in 12-39 year-olds [2] and a meta-analysis finding 15.7 per 100,000 in adolescent males after the second dose [3]. The cases are typically mild and self-limited [1, 2]. For clots, adenoviral vector vaccines carry a higher risk: VITT (vaccine-induced thrombotic thrombocytopenia) occurs at about 3.83 cases per million doses for the J&J vaccine [5], and cerebral venous sinus thrombosis at about 16.34 per million for AstraZeneca [5]. mRNA vaccines have much lower clot signals, with TTS incidence of 0.00855 per million [5]. A self-controlled case series found a small increase in venous thromboembolism after mRNA doses (IRR 1.19-1.22) but a much larger increase after COVID-19 infection itself (IRR 19.36) [4].
For a former IV opioid user, the mechanisms the retrieval cannot see are the ones that matter most. Chronic hepatitis C or HIV co-infection alters immune responses to vaccination. Endothelial damage from prior IV use could interact with the spike protein's known effects on vascular integrity. The cumulative inflammatory load from repeated infections and adulterants may amplify the inflammatory response to any vaccine. These are documented mechanisms outside this retrieval, but no study in these records tests them in this population.
My call: the evidence does not exist to answer this question. The general-population myocarditis and clot risks are small but real, concentrated in young males for mRNA vaccines and in women under 60 for adenoviral vaccines. Whether those risks are larger or smaller in someone with a history of IV opioid use is unknown. The most informative study design - vaccinated versus unvaccinated former IV users, followed for hard outcomes - has never been run.
