Does Atorvastatin cause dementia

The short version: the evidence does not show that atorvastatin causes dementia, but the question has never been properly studied.

The evidence you have here is almost entirely about the opposite question — whether atorvastatin might protect the brain. A 2025 Swedish registry study of 3,074 people with dementia found that atorvastatin users had a slightly higher MMSE score at diagnosis (21.6 vs 20.9 for non-users), but the difference was not statistically significant (p=0.475) [1]. A 2005 pilot trial in mild-to-moderate Alzheimer's patients found atorvastatin users scored about 3.5 points better on the ADAS-cog at 6 months than placebo [2]. The only evidence that even raises the possibility of harm is a 2003 case-report review that describes one patient on atorvastatin who developed "serious brain fog and cognitive impairment" within 5 days, which resolved 3 days after stopping [4]. That is a single case report — a signal worth noting, not proof of causation.

Here is the problem: no study in this retrieval compared dementia rates in a large group of atorvastatin users against a true untreated control group over many years.
