A 1985 randomized crossover trial is the cleanest evidence we have. Forty-six men with normal blood pressure drank regular beer, then switched to low-alcohol beer. Cutting alcohol by about 80% lowered systolic pressure by roughly 4 points, and blood pressure climbed back within two weeks of resuming regular beer [1]. The effect held even after accounting for the small weight loss, which means alcohol itself is the pressor, not the calories or the hops [1].
The mechanism fits what we know about alcohol and the nervous system. Alcohol dampens heart rate variability, which is a sign the body's brake on sympathetic drive is weakening [7]. That shift toward sympathetic overdrive is exactly what pushes blood pressure up over time. The 1985 trial showed the effect in men with normal pressure, so this is not a problem only for people already diagnosed with hypertension [1].
The evidence is old but it is direct and it is randomized. Nobody has run a modern trial that overturns it. The burden of proof sits with anyone claiming beer is neutral for blood pressure, and the 1985 data already met that burden in the other direction.
