The short version: no. They share one drug (emtricitabine) and differ in the other, and that difference is the whole point.

Truvada is emtricitabine plus tenofovir disoproxil fumarate (TDF). Descovy is emtricitabine plus tenofovir alafenamide (TAF). Same job, different second drug.

The difference that matters to you is kidney and bone. TAF was designed to deliver the same antiviral punch at a lower dose with less exposure to the rest of the body. In treatment trials, TAF held viral suppression about as well as TDF, roughly 92 in 100 versus 90 in 100 [3], while bone density at the hip fell about 0.7% on TAF versus about 3% on TDF [2][3]. Kidney markers moved less on TAF too [3].

TruvadaDescovy
Second drugTDF 300 mgTAF 25 mg
Viral suppression~90 in 100 [3]~92 in 100 [3]
Hip bone loss, 48 wk~3% [2]~0.7% [2]
Kidney signalmore creatinine rise [3]less [3]

For PEP specifically, the honest gap stands: the only PEP study here used a TAF pill bundled with two other drugs, and it was observational [1]. Nothing here tested Descovy alone as a PEP backbone.

My call: they do the same job, but Descovy is the gentler version on kidney and bone, and that advantage is proven in treatment, not in PEP. Confidence: high that they differ; moderate that the difference favors Descovy for a short PEP course.
