The short version: yes, the evidence is clear that H. pylori causes peptic ulcers, and the link is one of the best-established causal relationships in gastroenterology.
The evidence is strong and consistent across decades. About 95% of patients with duodenal ulcers and over 80% of those with gastric ulcers are infected with H. pylori [1, 6, 8]. The infection precedes the ulcer, and eradicating the infection dramatically changes the course of the disease. In controlled trials, ulcer recurrence drops from about 54% in those who remain infected to about 6% in those who are successfully treated [4]. A large cohort of 29,000 patients found that delaying eradication therapy raised the risk of recurrent ulcers by a factor of 3.55 and the risk of ulcer-related complications by a factor of 6.14 [5].
The mechanism is well understood. H. pylori colonizes the stomach lining, triggers chronic inflammation, disrupts the normal regulation of acid and gastrin, and damages the protective mucus layer [8]. Strains carrying the cag pathogenicity island cause more severe tissue damage and higher ulcer risk [2]. The infection does not cause ulcers in everyone — about 10 to 15% of infected people will develop an ulcer over 10 to 20 years [8] — but when an ulcer is present and no other cause like NSAIDs is involved, H. pylori is almost always the reason.
The evidence also shows that eradicating H. pylori prevents ulcer recurrence and rebleeding. After successful eradication, rebleeding rates drop to about 0.22% per patient-year [7]. One study found that eradication after surgical closure of a perforated ulcer reduced recurrence from 70% to 18.6% [3]. The benefit is large and reproducible across populations and study designs.
My call: H. pylori infection is a proven cause of peptic ulcer disease. Eradication therapy reliably prevents recurrence. Confidence: high.
