What changes in diet, apart from weight reduction, can help arthrpain
The evidence for diet changes reducing arthralgia is real but thin, mostly in people with inflammatory arthritis, and it comes with caveats about study design and who paid.
The most studied approach is the Mediterranean diet and long-chain omega‑3 fats (fish oil). A review of rheumatoid arthritis (RA) trials found modest improvements in pain and joint counts from a Mediterranean diet, with the best fish oil studies showing reductions in tender and swollen joints and less need for NSAIDs [1]. A separate review of knee osteoarthritis (OA) linked higher Mediterranean diet adherence to lower OA prevalence, though that is an association, not proof [2]. For fibromyalgia (FM), a 2010 review reports that uncooked vegan diets improved pain and stiffness in small, short-term studies, while a vegetarian diet alone failed to beat the drug amitriptyline [3]. One large placebo‑controlled trial of omega‑3s for breast‑cancer‑related joint pain found no advantage over placebo [4].
Two major problems. First, nearly all the RA and fibromyalgia trials were small (fewer than 60 people per group) and short (3–6 months) [1][3]. Second, who paid matters: the fish‑oil industry and commercial supplement makers funded many of these studies, and the one placebo‑controlled trial that found no benefit was run by the SWOG cancer research network, not by the industry [4]. A meta‑analysis of Mediterranean diet effects on diabetes (a proxy for its general anti‑inflammatory power) found modest improvements but noted most studies were short and funded by food‑industry affiliates [5].

What the evidence does not cover: any of these diets for common mechanical joint pain without underlying inflammatory disease—the kind most people have. The studies enrolled people with diagnosed RA, OA, or FM; there are no trials testing these diets in everyday arthralgia or general joint stiffness. The mechanism suspected (reducing systemic inflammation through omega‑3s and polyphenols) is plausible, but without head‑to‑head data, it is a guess.
My call: For someone with inflammatory arthritis, switching to a Mediterranean diet and adding fish oil (about 3–5 g/day) may reduce pain and the need for pills—the evidence is moderate but not zero. For fibromyalgia, an uncooked vegan diet helped in two small uncontrolled studies, but the evidence is weak. For ordinary joint aches with no diagnosed disease, there is no published trial; going by the inflammatory mechanism is a reasonable low‑risk bet, but it is not proven therapy. Confidence: moderate for inflammatory arthritis, low for everything else.
