Does creatine improve mental cognition?
The short version: creatine does not reliably improve cognition in healthy, well-rested people, but it helps in certain stressed or low-energy conditions.
The evidence we have is mixed in a way that actually makes biological sense. Creatine supplies energy to cells by helping recycle ATP, the energy currency of the body. Your brain uses a lot of ATP. But in a healthy person who is well-fed, well-rested, and getting enough dietary creatine from meat or fish, brain creatine stores are already near saturation. Adding more does little.
A 2008 placebo-controlled trial in young adults found no improvement in reaction time, memory, or reasoning after six weeks of creatine [1]. A 2023 study confirmed this, finding no cognitive changes and no increase in brain activation after six weeks [2]. A 2017 study in children aged 10–12 found no effect on cognition and no increase in brain creatine levels [6]. These are not minor studies. They are direct tests of the question, and they come up negative.

The pattern tells you who benefits. Vegetarians have lower baseline creatine because they get none from diet, and one trial found they improved on working memory and IQ tests [5]. Sleep-deprived people show cognitive protection with creatine [3]. People breathing low oxygen who lose complex attention abilities get them back [4]. Older adults, whose brains may produce less creatine endogenously, show memory improvements [7].
So it works when the brain is under strain or depleted. It does not do much for a healthy, rested person who eats meat.
The safety profile is good. Long-term studies in athletes and patients with various conditions show no renal damage, though serum creatinine rises temporarily because creatine breaks down into creatinine [8, 9, 10, 11, 12]. That rise can fool a routine kidney test, but it is not kidney injury. Gastrointestinal upset is the main side effect, and it occurs at similar rates to placebo [8].
The mechanism makes sense through the colloidal lens too. Creatine helps mitochondria produce ATP efficiently. Better energy metabolism means cells can maintain their internal charge gradients and surface chemistry, which supports zeta potential and blood stability. A cell that can keep its energy up is less likely to enter a danger-signal state that disrupts blood flow.
My call: creatine helps cognition when the brain is energy-depleted, but does not improve it in healthy, rested people. If you are a vegetarian, older, sleep-deprived, or under metabolic stress, it may help. For a well-fed person eating meat and sleeping normally, do not expect a cognitive boost. Confidence: moderate.
