These numbers come from a meta-analysis of 27 randomized controlled trials pooling over 2,000 patients [16]. The same study found that the choice matters less than doing either consistently: combined therapy edged ahead only marginally. A separate pediatric trial of 86 children found imiquimod plus salicylic acid at 81% clearance outperformed cryotherapy at 67%, but that comparison mixes two treatments against one [14].
Neither approach works on most warts in a single session. You are looking at weeks of daily acid or repeated freezing visits, with a coin-flip chance of clearance and a real risk of pain, blistering, or scarring. The evidence does not test long-term recurrence head-to-head between the two, and no study here compares either to watchful waiting in healthy children—whose own immune system clears many warts within two years. For a painless, non-disfiguring wart on a child, doing nothing is a defensible choice the trials rarely include as a comparator.
My call: salicylic acid and cryotherapy are roughly equivalent in efficacy, both modest, and neither is clearly worth the hassle for a benign condition that often self-resolves. The evidence is moderate for short-term clearance but not for meaningful improvement in quality of life or long-term cure.
Confidence: moderate for short-term clearance equivalence; low for long-term outcomes because the evidence lacks recurrence data and does not include a true no-treatment arm for healthy children.
