Curcumin, piperine and omega 3 together for inflamation?
The short version: the combination has plausible mechanisms and some biomarker evidence, but the human trials are small, short, and mostly test surrogate endpoints, not hard clinical outcomes like hospitalization or death.
The evidence for this combination comes from a handful of small trials. A 2025 randomized trial in 60 critically ill sepsis patients found that curcumin plus piperine lowered CRP from about 57 to 39 mg/L and ESR from 85 to 51 mm/hr compared to placebo [3]. That is a real biomarker improvement, but the trial was tiny, lasted only days, and mortality was similar between groups (10 vs 12 deaths) [3]. A 2025 protocol for a migraine trial plans to test a similar mix but has not reported results yet [1].
The piperine part is well-supported as a bioavailability booster. A small human pharmacokinetic study in just 3 people found that adding black pepper extended curcumin's half-life from about 1.4-3.0 hours to 3.7-5.3 hours and roughly tripled its 24-hour urinary excretion [2]. That matters because plain curcumin is notoriously poorly absorbed.
The omega-3 part is separate from the curcumin-piperine studies. A 2025 systematic review of nutraceuticals in rheumatoid arthritis found that fish oil (DHA/EPA) reduced disease activity scores in several small trials [5]. A 2019 crossover trial in 23 women with rheumatoid arthritis showed that 11 weeks of blue mussel intake raised erythrocyte EPA and DHA and shifted the fatty acid pattern [4]. The omega-3 index went from about 7.7% to 8.4% on the mussel diet [4].
What is missing matters more than what is here. No trial has tested all three together in the same people. No trial has tracked hard outcomes like hospitalization, heart attack, or death. The longest trial in this evidence set is 11 weeks [4]. The largest is 60 people [3]. The funding sources are not disclosed in these records, but the trials were run at university hospitals in Iran and Sweden, not by manufacturers.
The mechanism is plausible. Curcumin blocks NF-κB, the master switch for inflammation, and piperine helps it get into the blood. Omega-3s produce resolvins that actively turn inflammation off. Together they target different parts of the same inflammatory cascade. But plausible mechanism is not proof of clinical benefit.
