Can diet changes do better than stations
The short version: diet changes can match or beat statins for preventing cardiovascular death, but the evidence is observational, not from head-to-head trials.
The evidence for diet is strong but comes from a different kind of study. The Mediterranean diet cuts cardiovascular mortality by roughly 50% compared to a Western diet in large observational cohorts [1, 8, 13]. A meta-analysis of healthy eating indexes found a 23% reduction in cardiovascular death for people who ate well [6]. The Portfolio diet (plant-based, high in fiber, soy, and nuts) lowered cardiovascular mortality by 12% in a 2025 US cohort [5]. These effects are in the same range as statins for primary prevention, where the mortality reduction is around 7-15% [8].
But there is a catch. The diet studies are observational, not randomized trials. People who eat well also exercise more, smoke less, and have better access to healthcare. The statin trials are randomized, so the comparison is not fair. No large trial has directly compared a diet change to a statin for preventing death. The Women's Health Initiative tried a low-fat diet in 49,000 women and found no reduction in heart disease or death [9]. That failure may be because the diet was low-fat, not Mediterranean, or because the intervention was too weak.

The mechanism matters. Diet works through multiple pathways: lowering inflammation, improving blood sugar, reducing blood pressure, and stabilizing the colloidal properties of blood. Statins mainly lower LDL cholesterol. A 2022 study found that men with non-HDL cholesterol around 142-144 mg/dl had the lowest death rates, with higher mortality below that level [15]. That suggests aggressively lowering cholesterol with drugs may harm some people, while diet does not carry that risk.
My call: for a healthy person without existing heart disease, a Mediterranean-style diet is likely as good as or better than a statin for preventing cardiovascular death, but the comparison has never been tested in a proper trial. For someone who has already had a heart attack, statins have stronger evidence. Confidence: moderate for the diet comparison in primary prevention, high for statins in secondary prevention.
