The short version: rabies is nearly 100% fatal, and for dogs the vaccine is the only proven protection, but the evidence here says boosters every 2–3 years may be enough, not yearly.

The question is whether your dog needs the vaccine at all, and the honest answer is yes if rabies exists where you live. The disease is not a mild risk. In one Chinese province, 1,022 people died of rabies over nine years, and 96% of the biting dogs were unvaccinated [4]. In Tanzania, cutting vaccination coverage from 90% to 21% of dogs coincided with rabies incidence jumping from 0.19 to 9.4 cases per 1,000 dogs [3]. The vaccine works. A Japanese study found that after two or more doses, about 97 in 100 dogs still had protective antibodies at 12 months, and about 93 in 100 at 36 months [1]. That same study concluded annual boosters are reasonable but that every 2–3 years could maintain protection [1].

The evidence is thin on harm. One small study of 44 dogs with immune-mediated anemia found 2 of 22 vaccinated dogs had possible reactions, but that is a sick-dog population, not a healthy one [19]. The Japanese data put vaccine adverse events at 0.6 per 10,000 doses [1]. No long-term safety trial comparing vaccinated to unvaccinated dogs appears in this retrieval. That gap matters, but it does not change the calculus: rabies kills almost every unvaccinated dog it infects, and the vaccine is the only thing standing between your dog and that outcome.

My call: vaccinate your dog against rabies, but ask your vet about a 3-year booster schedule rather than the automatic annual one. Confidence: high on the vaccine's necessity, moderate on the optimal booster interval.
