The short version: paracetamol reduces pain, but the evidence that it worsens infections is thin and indirect.
The retrieved studies mostly test pain relief, not infection outcomes. They show paracetamol works for postoperative pain [1, 2], but not better than other options [8]. One 2025 pediatric review flags a real concern: ibuprofen use in respiratory infections may link to complicated pneumonia, and it advises caution with NSAIDs during infections [7]. That is about ibuprofen, not paracetamol.
The mechanism is plausible. Paracetamol blocks COX enzymes centrally [15, 16], and fever is part of the immune response. Suppressing it could theoretically slow clearance. But none of these studies measured infection severity or duration as an endpoint. The evidence simply does not answer the question.
