What is the evidence for rsv vaccination in elderly. Benefits and risks

The short version: the RSV vaccines for elderly adults reduce lab-confirmed lower respiratory tract infections over one season, but no trial in the evidence shows a reduction in all-cause mortality, and long-term safety data is absent.

The evidence comes from two Phase III trials funded by manufacturers. The RSVPreF3 OA (GSK) trial randomized about 25,000 adults 60+ and reported 67–83% efficacy against RSV-related lower respiratory tract infection [6]. The Nano-RSV trial randomized 400 adults 60+ and reported a 46% reduction in RSV LRTI incidence (8.5% vs 15.2% in placebo) and a halving of RSV-related hospitalizations (3.2% vs 6.5%) [8]. Both trials used hard clinical endpoints, which is better than surrogate antibody titers. But neither trial compared vaccinated to truly unvaccinated people over multiple seasons, and neither reported all-cause mortality or serious adverse events beyond the short follow-up window.

The safety data is thin. The Nano-RSV trial reported injection-site and systemic reactions at rates similar to placebo, with 2 severe adverse events in the vaccine group and 1 in placebo [8]. That is a single trial with 400 people, far too small to detect rare harms. The Italian cross-sectional study of 453 vaccinated elderly reported high trust in vaccines but no safety follow-up at all [4]. No study in the evidence followed participants for longer than one RSV season. No study compared vaccinated to unvaccinated over multiple years. The most informative safety design was never run.

My call: the vaccines likely reduce RSV-related respiratory illness over one season in elderly adults, but the evidence does not show a mortality benefit, does not include long-term safety data, and comes from manufacturer-funded trials with no multi-year unvaccinated comparison group. Confidence: moderate for short-term efficacy against mild illness, low for safety beyond the first season.
