What exercises are most effective for arthritis knee pain
The best evidence points to specific types of exercise, not just "any movement."
The studies here consistently show that the type of exercise matters more than its intensity. Several trials found that neuromuscular and sensorimotor training — exercises that challenge balance, coordination, and joint position sense — consistently beat simple quadriceps strengthening for pain reduction and function [2, 9, 10]. One trial in 66 people with knee osteoarthritis found that adding sensorimotor training to traditional exercises dropped pain scores from about 6.8 to 2.6 on a 10-point scale, while exercise alone only reached 4.4 [9]. That is a meaningful extra benefit.
For knee osteoarthritis specifically, the evidence favors:

The conventional "just strengthen your quadriceps" advice is incomplete. Hip abductor training mattered in a 2022 trial of 40 patients: the group that did it got significant pain relief while the control group did not [1]. A 2019 trial with 59 people found that neuromuscular training beat quadriceps training for gait speed, stride length, and WOMAC scores [10]. A 2018 trial found that arm ergometer plus resistance training actually gave better knee pain relief than treadmill or cycle — the benefit seems to come from the resistance itself, not from loading the joint [6].
The one study that directly compared exercise intensity found no difference between high and low intensity cycling [4]. You do not need to push hard. You need to do the right movements.
My call: the most effective group of exercises for knee osteoarthritis is a mix of hip abductor strengthening, sensorimotor/balance work, and non-weight-bearing resistance training. Quadriceps work alone is a weaker strategy. Confidence: moderate — the studies are small but the pattern is consistent and biologically plausible.
