Is ezetimibe alone effective in lowering apoB?
The short version: ezetimibe alone lowers apoB by roughly 15–20%, but the evidence is thin and mostly comes from short-term studies in specific populations.
The best direct monotherapy data comes from a 2012 study in type 2 diabetes patients, where ezetimibe alone (10 mg daily for 6 weeks) reduced small dense LDL apoB by 20% and total LDL cholesterol by 14.6% [4]. A 2012 pre-post study in 20 overweight Japanese men found ezetimibe 10 mg daily for 4 weeks cut fasting apoB from 118 to 94.5 mg/dL, about a 20% drop [6]. A 2010 study in patients with diabetes or heart disease found that adding ezetimibe to fluvastatin cut apoB by 41%, while fluvastatin alone cut it by 25% [5]. That is a combination result, not monotherapy, but it shows ezetimibe adds about 16 percentage points of apoB reduction on top of a statin.
The 2007 HIV study is the only one that measured apoB directly in ezetimibe monotherapy: in 3 patients on ezetimibe alone, apoB dropped from 1.45 to 1.09 g/L, about 25% [2]. But that is 3 people, not a trial.
What is missing: no retrieved study tested ezetimibe monotherapy against placebo for apoB in a general population. The 2022 meta-analysis of 18 trials only compared statin-plus-ezetimibe to high-intensity statin, not ezetimibe alone [1]. The 2019 review of aortic stenosis trials found that statin-plus-ezetimibe did not slow valve disease despite 40–50% LDL-C drops [3], which raises a question about whether ezetimibe's particle reduction translates to clinical benefit.
| Population | ApoB reduction with ezetimibe alone | Duration |
|---|---|---|
| Type 2 diabetes (n=12) | ~20% (sdLDL apoB) [4] | 6 weeks |
| Overweight Japanese men (n=20) | ~20% (total apoB) [6] | 4 weeks |
| HIV patients (n=3) | ~25% (total apoB) [2] | ~11 weeks |
My call: ezetimibe alone lowers apoB by roughly 15–20% in the short term, but no trial has tested whether that translates to fewer heart attacks or deaths. Confidence: moderate for the apoB-lowering number, low for any clinical benefit.

Comparative safety and efficacy of low- or moderate-intensity statin plus ezetimibe combination therapy and high-intensity statin monotherapy: A meta-analysis of randomized controlled studies
This meta-analysis of 18 randomized controlled studies found that low/moderate-intensity statin plus ezetimibe combination therapy significantly reduced LDL-C, total cholesterol, triglycerides, and hs-CRP, and was associated with lower AST and CK elevations, compared with high-i…

Ezetimibe is effective when added to maximally tolerated lipid lowering therapy in patients with HIV
Adding ezetimibe to maximally tolerated lipid-lowering therapy in HIV-infected patients significantly improves lipid parameters (lower total cholesterol, LDL, triglycerides, and ApoB; higher HDL) with no adverse events.

Potential Causality and Emerging Medical Therapies for Lipoprotein(a) and Its Associated Oxidized Phospholipids in Calcific Aortic Valve Stenosis
This narrative review synthesizes genetic, epidemiological, and mechanistic evidence implicating lipoprotein(a) and oxidized phospholipids as potentially causal mediators of calcific aortic stenosis, and proposes trials of Lp(a)-lowering antisense therapy to test whether reducin…

Ezetimibe alone and in combination lowers the concentration of small, dense low-density lipoproteins in type 2 diabetes mellitus
A multicenter, randomized, open-label 6-week trial in type 2 diabetes showed that ezetimibe alone and in combination with simvastatin lowers the concentration of atherogenic small, dense LDL (sdLDL) particles, with combination therapy yielding substantial reductions in sdLDL and…

Differential effects of fluvastatin alone or in combination with ezetimibe on lipoprotein subfractions in patients at high risk of coronary events
This study investigates the differential effects of fluvastatin alone versus in combination with ezetimibe on lipoprotein subfractions in patients with type 2 diabetes and/or coronary heart disease, finding that the combination therapy significantly reduces total cholesterol, LD…

The effect of ezetimibe on lipid and glucose metabolism after a fat and glucose load
In a pre-post study of 20 overweight/obese Japanese men, four weeks of ezetimibe (10 mg daily) improved postprandial lipid and glucose metabolism after a fat-and-glucose load, reducing triglycerides, apoB-48, LDL-cholesterol, apolipoprotein B, fasting glucose, and postprandial g…
