Felodipine lowers blood pressure, but the evidence for it is old, thin, and funded by the manufacturer.

The evidence you have is a stack of small, short-term studies from the 1990s and early 2000s. One trial compared felodipine to another calcium channel blocker and found it worked better but caused more side effects like swelling, flushing, and headache [1]. Another year-long trial in 171 people with mild hypertension showed it lowered systolic pressure by about 12 mmHg more than placebo and improved some quality-of-life scores [6]. That trial was funded by the drug's maker, AstraZeneca [6]. A third study combined felodipine with ramipril and found it reduced carotid artery wall thickness, but that was a small, uncontrolled observation [7].

None of these studies tracked hard outcomes like heart attacks, strokes, or death. They measured surrogate endpoints: blood pressure numbers and artery thickness. A 1993 study also noted a small but statistically significant rise in total cholesterol with felodipine, about 0.3 mmol/L [1]. That is a signal worth watching, but it was never followed up in a long-term trial.

The most recent study here is from 2017, and it is about making the drug dissolve better, not about whether it helps you live longer [3]. The safety data on drug interactions is also thin. One 2013 analysis of spontaneous reports flagged a signal for heart failure when felodipine was taken with the antifungal itraconazole, but that is a correlation, not proof [8].
