Can fibromyalgia symptoms mimic MS on MRI or neurologic exams?

Fibromyalgia is a central sensitization state, not a demyelinating disease [6, 11]. The 2016 Neuroscience review details that fibromyalgia involves enhanced pain processing in the insula and decreased descending inhibition, but it produces no structural brain lesions [6]. No study in your evidence examines MRI findings in fibromyalgia patients. The MS peptide review [13] describes myelin-specific pathology—increased citrullinated MBP, arginine methylation, altered phosphorylation—that is completely absent in fibromyalgia. A fibromyalgia patient should not show the characteristic demyelinating plaques (periventricular, juxtacortical, infratentorial) that define MS on MRI. Your hypothesis is mechanistically sound: the two conditions share no common lesion-forming pathology.
The clinical mimicry comes from overlapping symptoms—widespread pain, fatigue, cognitive complaints—not from shared MRI findings. The 1990 ACR criteria require 11 of 18 tender points plus widespread pain [9], but they explicitly exclude organic neurologic disease. No study in your evidence compares neurologic exam findings between the two populations. The clinical overlap is real (both can present with numbness, tingling, weakness, blurred vision), but when a patient has a normal MRI with no demyelinating lesions, the label "fibromyalgia" is a default, not a mimic.
My call: fibromyalgia does not produce MS-typical MRI lesions. If an MRI shows no demyelination, the diagnosis is not MS. Confidence: high—the mechanism is settled, but the clinical mimicry question needs a head-to-head MRI study comparing fibromyalgia to MS patients, which your evidence does not provide.
