What is the flu jab in the UK like in terms of side effects

The short version: the UK flu jab’s side-effect profile is mostly mild and short-lived, but the evidence on rare serious harms is thin because the most informative study design—vaccinated versus truly unvaccinated over time—is almost never run.

The evidence retrieved covers UK-specific safety data for the live attenuated nasal spray (LAIV) used in children. A large English cohort of 11,463 high-risk children (mostly asthmatics) found no significant increase in all-cause hospitalisation after LAIV compared to matched unvaccinated controls, and no cases of Guillain-Barré syndrome, Bell’s palsy, or narcolepsy were observed [13]. That sounds clean, but the study was observational, not randomised, and the authors note that residual differences in baseline health between vaccinated and unvaccinated children could mask a signal [13]. The study was also too small to detect a very rare event like GBS if the added risk were below 1 per 1,500 patient-years [13]. That is a real gap: the 1976 swine flu vaccine caused about 10 extra GBS cases per million doses [1], and later seasonal flu vaccines have shown a smaller but measurable signal (about 1 extra GBS case per million) [1]. The UK data simply cannot rule out a risk of that size.

For adults, the retrieved evidence is mostly from non-UK studies and manufacturer-funded trials. A 2006 Japanese study of healthcare workers reported 20 adverse reactions in 237 vaccinated staff—19 local, 2 general malaise—but no severe events [12]. That is a tiny sample with no long-term follow-up. The broader picture from the literature is that the inactivated flu shot causes injection-site pain, redness, and mild systemic symptoms (fever, fatigue) in a minority of recipients for 1-2 days. Serious harms like anaphylaxis are very rare but real. The key point: the evidence that exists is short-term and passive. No long-term safety registry follows UK flu vaccine recipients against an unvaccinated comparator. The burden of proof has not been met for rare or delayed harms.
