Is the folic acid in bread worth the risk if yoj are pre cancerous
For someone with pre-cancerous lesions, the folic acid in bread is a real risk, not a theoretical one.
The evidence shows a clear pattern: folic acid can feed existing cancer cells. A 2008 review of the Aspirin/Folate Polyp Prevention Study found that people taking folic acid supplements had a 67% higher risk of advanced colorectal adenomas and more than double the risk of multiple adenomas [4]. That study tested 1 mg/day, far above what bread adds, but the mechanism is the same. A 2013 lab study showed that high concentrations of folic acid silence tumor suppressor genes PTEN, APC, and RARbeta2 through DNA hypermethylation in breast cancer cells [6]. This is not a minor effect. It is a direct epigenetic mechanism that could accelerate progression from pre-cancer to cancer.
The benefit side is weaker than advertised. Fortification clearly prevents neural tube defects in developing fetuses, with a 30% reduction in Brazil [3] and 30-50% in the US and Canada [2]. But for an adult with pre-cancer, that benefit does not apply. The same review that noted the adenoma risk also described a U-shaped relationship: cancer risk is lowest around 300-400 mcg/day of folate and rises at intakes above 1 mg/day [2]. Fortified bread alone adds about 159 mcg/day for women of child-bearing age [1], and total intake from food plus fortification in the US averages 323 mcg/day for non-supplement users [5]. That is within the safe range for most people, but the margin is thin. Children aged 2-8 years already exceed the upper tolerable limit after fortification, with 24% of 2-3 year olds above it [1].
The question is whether you want to add any folic acid at all when you already have cells that might be one mutation away from malignancy. The evidence says no. The mechanism is plausible, the human trial data shows harm at supplement doses, and the fortification levels are higher than originally predicted [1]. You can get folate from natural food sources like leafy greens, which come with the full nutrient package and no risk of feeding pre-cancerous cells.
My call: avoid folic acid-fortified bread if you are pre-cancerous. The risk of accelerating existing lesions outweighs any benefit for an adult who is not pregnant. Confidence: moderate. The human trial data on adenoma recurrence is strong, but the exact dose-response for bread-level fortification in pre-cancerous individuals has not been studied directly.
