The short version: the evidence that folic acid in food prevents neural tube defects is real, but the story of what happens when everyone gets extra folic acid, whether they need it or not, is far less clean than what you are told.

The core benefit is real. Randomized controlled trials showed that folic acid supplementation can prevent up to two-thirds of neural tube defects (NTDs), and after the US and Canada made it mandatory in enriched flour in 1998, NTD rates dropped 30% to 50% [2]. That is a genuine public-health win for a specific window: women who become pregnant and need folate in the first weeks before they know they are pregnant.

But here is where it gets complicated. The evidence shows a U-shaped relationship between folate intake and cancer risk, with the lowest risk around 300–400 mcg/day and elevated risk when intake falls below 150 mcg or above roughly 1 mg per day [2]. A meta-analysis of 10 randomized trials found a borderline-significant increase in overall cancer with folic acid supplementation, driven mostly by prostate cancer [2]. The synthetic folic acid used in fortification, which is not the same as natural folate from food, can also show up in the blood unmetabolized. One study in the evidence found that unmetabolized folic acid was associated with reduced natural killer cell cytotoxicity, a key part of immune surveillance against cancer and virus-infected cells [1]. And in a 2025 mouse study, prenatal exposure to a 5-fold excess of folic acid altered gene expression and DNA methylation in the newborn cortex, affecting mitochondrial function, protein translation, and glutathione pathways [3].

The real question is who gets what dose. The recommended intake for pregnancy is 600 mcg daily of dietary folate equivalents [1], and the tolerable upper limit for folic acid from supplements and fortification is set at 1000 mcg daily, mainly to avoid masking a B12 deficiency [1]. The problem is that fortification hits everyone, not just pregnant women. A modeling study in the US found that after mandatory fortification, average folic acid intake from food alone among non-supplement users rose to about 190 mcg daily [6]. But supplement users get an extra 100–400 mcg or more. A systematic review found that women of childbearing age who take a folic acid supplement in countries with mandatory fortification nearly all exceed the 1,000 mcg upper limit [4]. Meanwhile, in Australia, where bread flour is fortified, children aged 2–8 years had the biggest increases above the upper limit: 5% before fortification, 24% after [5]. You are blasting a synthetic folate into the entire population to catch the first trimester window of a fraction of them, and the long-term impact on immune balance, cancer risk, and childhood development has never really been studied the way you would want.

The absolute best evidence of benefit is going to women who are trying to conceive or in their first trimester. For everyone else, the risk-reward balance is more uncertain, and it is not irrational to prefer natural folate from vegetables, beans, and sources like that.

My call is that for someone who could become pregnant, 400 mcg daily for NTD prevention is worth it, but the mass fortification of the entire food supply is an experiment with incomplete safety data and there is a plausible biological mechanism for harm at excess doses. Confidence: moderate.
