Have there been serious side effects from bempedoic acid / ezetimibe and is it important to take for heart health after heart surgery?
The short version: these drugs lower your LDL numbers on the lab sheet, but the biggest trial on bempedoic acid found it did not lower your chance of dying from heart disease.
The CLEAR Outcomes trial, which is the key evidence here, was a randomized placebo-controlled study of 13,970 statin-intolerant patients followed for a median of 40.6 months. Bempedoic acid reduced the composite heart event rate (heart attack, stroke, revascularization) by about 13% compared to placebo, with an absolute risk reduction of 1.6 percentage points [4]. But when the researchers looked at death from cardiovascular causes separately, the difference vanished: 3.8% in the bempedoic acid group versus 3.7% in the placebo group, a hazard ratio of 1.04 with a confidence interval crossing 1.0 [2]. That means the drug did not prove it keeps people alive longer. The same pattern holds for ezetimibe: in the IMPROVE-IT trial, adding ezetimibe to simvastatin versus simvastatin alone produced no difference in cardiovascular death (HR 1.0, CI 0.89-1.13, p=1.00) [2].

The known side effects from the evidence are concrete. Bempedoic acid raises uric acid and increases gout risk by about twofold (relative risk 2.05, 95% CI 1.81-2.33) [5]. The combination of bempedoic acid and ezetimibe was described as "well tolerated" in a 12-week trial [1, 3], but that is a short window; long-term safety data beyond 40 months does not exist for this combo. Muscle symptoms, the reason many patients seek these alternatives, were lower with statin + ezetimibe combination than with high-intensity statin alone (0.7% vs 5.7% incidence in the SaveSAMS trial) [4].
After heart surgery, the question is whether lowering LDL with these drugs changes what happens to you, not just your lab results. The evidence says the drugs reduce non-fatal events but have not been shown to extend life. An editorial review notes that bempedoic acid's benefits are clearest for patients who genuinely cannot tolerate statins and still need additional LDL reduction [5]. If you can tolerate even a low-dose statin, the data for that class is much stronger across decades of trials.
My call: bempedoic acid and ezetimibe lower LDL but have not been proven to reduce cardiovascular death. The decision depends on whether you tolerate statins and how much weight you put on preventing non-fatal events versus the absence of a mortality benefit. Confidence: moderate — the outcome trials are large and randomized, but their own data shows no survival advantage, and long-term safety on the combination is thin.
