Question explored with the scientific record
Heart health Nattokinase
Nattokinase moves lab markers in the right direction. No retrieved study shows it prevents a heart attack, stroke, or death.
The evidence is small and mostly on surrogate endpoints, meaning lab values and artery-wall measurements rather than heart attacks or deaths. A 56-day placebo-controlled trial of 60 people with high cholesterol found total cholesterol down about 10 percent and LDL down about 11 percent [1]. A 90-day study of 30 people with thickened carotid arteries found plaque shrank more when nattokinase was added to standard cholesterol drugs [1]. The largest dataset, 1,062 people on 10,800 FU daily for a year, was retrospective, meaning it looked backward at existing records with no true untreated comparison group [1]. That design cannot prove cause.
The clot-dissolving mechanism is real in the lab. Nattokinase is a serine protease that breaks down fibrin, the protein mesh of a clot, and blocks the inhibitor of tissue plasminogen activator, the body's own clot-buster [2]. In rats fed it for 14 weeks, clot breakdown matched an intravenous clot-busting drug and fibrinogen fell sharply [4]. In healthy young men, a single dose barely moved fibrinogen but nudged clotting measures [5]. Human fibrinogen drops of 7 to 10 percent have been reported across groups [3]. Whether lower fibrinogen translates into fewer heart attacks is not shown here.
| Study | People | Design | Main result |
|---|---|---|---|
| Cholesterol trial [1] | 60 | Placebo, 56 days | LDL down 11% |
| Carotid plaque trial [1] | 30 | Added to statins, 90 days | Plaque down 36.6% |
| Large cohort [1] | 1,062 | Retrospective, 1 year | LDL down 18.1% |
| Rat thrombosis [4] | 8 per group | Animal, 14 weeks | Clot volume down to 51.4% |
The bleeding risk is the one to weigh. Anything that thins clots can add to blood thinners, and no retrieved record gives a bleeding rate in people taking it alongside aspirin or warfarin. That gap matters for anyone on those drugs.
My call: reasonable to try for cholesterol and clot markers if you understand no trial has shown it prevents a heart attack, and talk to your doctor first if you take blood thinners. Confidence: low.

Natural Remedies Lower Elevated Fibrinogen Levels in the Prevention of Atherosclerosis
This study reviews evidence that elevated plasma fibrinogen levels are both a consequence and predictor of cardiovascular disease (CVD), and discusses various natural remedies that can effectively lower these levels to prevent CVD.

The Effect of Dietary Bacillus Natto Productive Protein on in vivo Endogenous Thrombolysis
In rats, 14 weeks of dietary Bacillus natto productive protein (BNPP) enhanced endogenous thrombolysis dose-dependently; 1% BNPP matched 0.2 mg/kg i.v. t-PA and lowered plasma fibrinogen, while argatroban was not additive.
