The short version: the evidence you provided does not test whether creatine plus alpha-lipoic acid helps long COVID patients, and the one study that is even close is about a different question entirely.
The evidence you gave me contains nothing that tests your hypothesis. Not one record. The closest is the 2014 platelet study [1], which shows alpha-lipoic acid reduces platelet clumping in a test tube. That is a plausible mechanism for long COVID—blood sludging and microclots are part of the picture—but it is not a clinical trial in long COVID patients. The rest of the alpha-lipoic acid records study ear infections in guinea pigs [2], pancreatic injury in rats [3], gestational diabetes [4], nerve injury in rats [5], spinal cord injury in rats [6], chicken testes [7], and breast cancer cells [12]. None of them test your hypothesis. The creatine records are not in this evidence set at all.
The 2024 bivalent vaccine study [15] is the only record that tests an intervention against long COVID symptoms, and it found about a 40% lower risk of having three or more symptoms at six months in vaccinated people compared to unvaccinated people. That is a different intervention, a different mechanism, and it does not tell you whether creatine plus alpha-lipoic acid works. The 2023 Korean cohort [17] found that 71% of people still had long COVID symptoms at 24 months and that post-infection vaccination did not significantly reduce prevalence. That is a natural history study, not a treatment trial.
