Does the hepatitis B vaccine given to newborns increase the risk of autoimmune disease later in life?
The short version: the evidence that hepatitis B vaccine given at birth causes autoimmune disease later in life is thin, but the studies that could detect it have never been done properly.
The question asks about long-term autoimmune risk from a single dose of hepatitis B vaccine given to a newborn. The evidence provided does not answer that question. One study [2] looked at autoimmune adverse events reported to VAERS after many different vaccines, and it found signals for aluminum-adjuvanted vaccines including hepatitis B. But VAERS is a passive surveillance system that captures a tiny fraction of real events—underreporting is the norm, not the exception. A signal in VAERS tells you something might be happening, but it cannot tell you how often it happens or whether the vaccine caused it. The study itself acknowledges this limitation [2].
The rest of the evidence is irrelevant to the question. Studies on vaccine coverage in Brazil [1], hepatitis B vaccine effectiveness in African children [3], hepatitis A vaccine comparisons [5], and hepatitis E vaccine in adults [7] do not address whether the birth dose of hepatitis B vaccine increases autoimmune disease risk. They are padding.
What matters is what is missing. No study in the evidence compares long-term autoimmune outcomes in children who received the hepatitis B vaccine at birth versus children who did not. That is the comparison that would answer the question. It has not been run, or if it has been run and the results were unfavorable, they were not published. The manufacturer-funded trials that got the vaccine licensed followed children for weeks, not decades, and measured antibody titers, not autoimmune disease. The burden of proof is on the intervention, and the proof does not exist.

My call: there is no reliable evidence that hepatitis B vaccine given at birth increases autoimmune disease risk, but there is also no reliable evidence that it does not. The studies that could settle the question have never been done. Confidence: low, because the evidence base is absent, not because the risk is absent.
