The short version: hot baths are a genuine medical tool for specific problems, but the research is scattered across animal studies, case reports, and small human trials, with almost no large-scale safety data.

The evidence for hot baths clusters into three areas. For venomous stings (stingray, catfish, weever fish), hot water immersion is the standard first-aid treatment. One prospective study of 22 stingray injuries found that hot water alone dropped average pain from 7.4 to 2.2 on a 10-point scale, with only a 4.5% infection rate [3]. Case reports on catfish and weever fish stings also report pain relief from hot water, though outcomes varied [5, 7]. The mechanism is heat denaturing the venom proteins, which is straightforward physical chemistry.

For chronic pain and inflammation, the picture is more mixed. A systematic review of spa therapy for knee osteoarthritis found that multicomponent hot water programs improved pain and function compared to no treatment, but the studies were highly variable in quality and the effects were not consistent enough to pool into a single number [14]. A small rat study showed that daily hot water immersion (42°C for 15 minutes over 7 days) reduced inflammatory markers TNF-α and IL-6 and lowered heart rate compared to a single exposure, suggesting the body adapts to regular heat stress [4]. A pilot study in 15 healthy men found that four hot spring baths over two weeks shifted gut bacteria toward more beneficial species and raised short-chain fatty acids, though this was a small, uncontrolled pre-post design [30].

The risks are real and understudied. The most concerning signal comes from a 1992 case-control study of 44 matched pairs in Louisiana, which found that hot baths during the first month of pregnancy were associated with a 31-fold increase in the odds of spina bifida (95% CI: 1.68 to 577.3) [26]. The confidence interval is extremely wide because the sample was small, but the finding is consistent with known teratogenic effects of maternal hyperthermia. A case report of sudden death in a young woman found in a hot bath identified three rare genetic variants linked to heart arrhythmias, suggesting that hot water can trigger cardiac events in susceptible people [28]. And the phenomenon of compulsive hot bathing in cannabinoid hyperemesis syndrome is well-documented, with patients spending hours in hot water to relieve vomiting [27, 29].

My call: For venomous stings, hot water immersion is well-supported and should be used. For chronic pain, the evidence is suggestive but weak. For pregnant women, the signal of harm is strong enough to warrant caution despite the small sample. For everyone else, the long-term safety data simply does not exist. Confidence: moderate for sting treatment, low for most other uses, and the pregnancy risk signal is too large to ignore even with low confidence.
