Should I get the RSV vaccine for my healthy newborn baby?

The short version: nirsevimab reduces RSV hospitalizations in healthy newborns, but the evidence is entirely from manufacturer-funded trials and short-term observational studies, with no long-term safety data.

The evidence you have is a stack of studies funded or co-authored by the manufacturer (AstraZeneca/Sanofi) [1, 4], using a monoclonal antibody, not a vaccine. The key trial, MELODY, was a 2:1 randomized placebo-controlled phase 3 study in 3,012 healthy infants [1]. It showed nirsevimab cut RSV-related medically attended lower respiratory tract infections by about 79% and hospitalizations by about 77% through day 511 [1, 4]. Real-world data from Italy, Spain, and Germany report 80-90% reductions in RSV hospitalizations after universal rollout [2, 3, 6, 7]. The number needed to immunize to prevent one hospital admission is 128 [8].

What is missing is long-term safety. The trials followed infants for 511 days at most [1]. No study compared vaccinated to truly unvaccinated children over years. The NIRSE-GAL study found that 15 of 45 breakthrough cases still needed ICU care [5]. The evidence does not include data on autoimmune outcomes, developmental delays, or all-cause mortality. The manufacturer is shielded from liability by the 1986 National Childhood Vaccine Injury Act, so the market force that would push for safer products does not exist.
