How do I know if I’ll pass the office test for topical-only?
The short version: the Lanindar test is the only evidence-backed way to predict your tolerance for topical-only cataract surgery, and it works well.
The strongest evidence here is the Lanindar test, studied in 716 patients [5]. Your surgeon shines a desk lamp in your dilated eye while lifting your upper eyelid with a finger. If you feel comfortable and do not squeeze your eye shut or pull away, you are Lanindar-positive. In the study, 621 of 716 patients (87%) passed. Of those, only 7 out of 621 (1.1%) needed to switch to a block or general anesthesia during surgery [5]. The test's specificity was very high, meaning a positive result strongly predicts you will tolerate the procedure [5].
The test matters because the main challenge of topical-only surgery is not pain but cooperation. Pain scores under topical anesthesia are very low. In one study of 22 patients, the average pain score was 0.9 out of 10, and no one needed extra anesthetic [3]. Another study of 40 patients found the highest pain came during lens implantation, averaging 1.0 out of 10 [2]. The real issue is whether you can hold still and keep your eye from squeezing shut while a surgeon works on it.
The factors that predict poor tolerance are known. A 2009 study of 203 patients found that women, younger patients, people from rural areas, and those with worse preoperative vision reported more pain [1]. A 2020 study of 1097 surgeries found that floppy iris syndrome, morbid obesity, and very dense cataracts increased surgeon stress, which correlates with patient difficulty [4]. If you have any of these, your odds of tolerating topical-only are lower.
The evidence does not include a head-to-head comparison of the Lanindar test against other prediction methods. It also does not tell you what to do if you fail the test. The study considered it unethical to expose Lanindar-negative patients to topical-only surgery, so we do not know how many of them would have actually managed [5]. The test may be overly conservative.
My call: ask your surgeon to perform the Lanindar test in the office. If you pass, your chance of tolerating topical-only surgery is about 99%. If you fail, plan for a block or sedation. Confidence: moderate.
