Do I need a COVID vaccine

The short version: for a healthy adult or child, the benefit is small and the safety data is incomplete.

The evidence shows the vaccines reduce severe outcomes in older adults and those with risk factors. In Turkey, two mRNA doses cut ICU risk by 99% in people over 65 compared to a single inactivated dose [16]. In Estonia, vaccinated people had a 68% lower risk of death in the year after infection [14]. But these are observational studies, not randomized trials, and the unvaccinated comparison group is never a true placebo.

For healthy children, the math is different. The infection hospitalization rate for children 5-14 is about 1 in a million [4]. Vaccine effectiveness against severe disease in children 5-11 ranges from 41% to 83% depending on the study [4]. Meanwhile, myocarditis after the second mRNA dose in adolescent males runs 6.7 to 15 per 100,000 [4]. The ACIP itself acknowledges safety signals for ischemic stroke after mRNA vaccines in older adults and Guillain-Barre in those 65+ [1].

The long-term safety data does not exist. No trial has compared vaccinated to unvaccinated over years with all-cause mortality as the endpoint. The liability shield means the manufacturer has no market incentive to run that study.

My call: for an older adult or someone with risk factors, the evidence supports vaccination. For a healthy child or young adult, the benefit is too small and the safety data too thin to justify it. Confidence: moderate for older adults, low for everyone else.
