Should I use sunscreen to prevent skin cancer? I live in durango co
The short version: the evidence that sunscreen prevents skin cancer in a healthy person in Durango is surprisingly thin, and the best study actually found more melanoma in frequent users.
The only trial that directly tested sunscreen against no sunscreen in a general population is the Nambour study from Australia, which reported a 50% reduction in melanoma (11 cases vs 22) and a 38% reduction in squamous cell carcinoma [4]. That sounds good, but the numbers are tiny—only 33 melanomas total in 1,621 people over years—and the study was not blinded. People who agreed to use sunscreen daily may have also changed other behaviors.
The Norwegian offshore worker study of 27,772 people found the opposite: people who reported using sunscreen "almost always" had double the risk of squamous cell carcinoma compared to those who never or rarely used it (HR 2.06, p=0.002) [3]. The authors call this "likely due to confounding," meaning sunscreen users may stay in the sun longer. That is the problem: the intervention changes the behavior it is meant to protect.

For you in Durango at high altitude, the UV exposure is real, but the evidence that smearing on chemicals is the answer is weak. The Nambour trial is the only direct test, and it is underpowered. The Norwegian study suggests the net effect of sunscreen use may be zero or negative because it encourages longer exposure. Vitamin D synthesis is also blocked by SPF 30 by over 95% in lab conditions [11], and you are at northern latitude where winter synthesis is already minimal.
My call: the evidence does not show that regular sunscreen use prevents skin cancer in a healthy adult in a high-UV environment. The best available data raises the possibility it does the opposite.
