If covid was so bad, why was the only recommendation from Drs to go to bed and take paracetamol?

The short version: the advice to rest and take paracetamol was the default because no drug had been proven to work for early COVID-19, and the evidence that did exist was weak, observational, and often contradicted.

In the first months of the pandemic, doctors had nothing solid to offer outpatients. The few early studies that claimed benefit for hydroxychloroquine plus azithromycin were small, non-randomized, and later contradicted by larger randomized trials that found no benefit [2, 8]. One commentary on the early evidence called it "weak at best" and pointed out that the most-cited French study had an odds ratio of 52 but a real risk ratio of about 5 — a massive inflation from a common-outcome bias [8]. By the time randomized controlled trials were done, the drug showed no effect on clinical status at 15 days [8]. So the "go to bed" advice was not a conspiracy; it was honest ignorance. The system had no early treatment to offer because the treatments that were proposed failed when tested properly.

What was not said is that the system also had no incentive to find cheap, unpatentable early treatments. The evidence that does exist for early intervention is almost entirely observational and industry-funded for later-stage drugs like remdesivir, which in one 2025 study of high-risk hospitalized patients showed no significant difference in mortality or oxygen need compared to control [16]. The burden of proof was never met for any early outpatient drug. The advice to rest and take paracetamol was the honest default of a system that had nothing better to offer — and that had not bothered to run the trials that might have found something.

My call: the advice was consistent with the evidence available at the time, which was thin and conflicted. Confidence: high that no proven early treatment existed; low that the system tried hard to find one.
