Question explored with the scientific record

If statins aren’t used, what alternative treatments reduce cholesterol?

Aug 30, 2026 · 27 sources used · OpenNeedle synthesis
The short version: none of the drugs here have been tested against a no-treatment control for hard outcomes like death or heart attack. They lower a number on a lab slip, and that is all.

The evidence you were given is almost entirely about ezetimibe and the PCSK9 inhibitors, and almost every study compares them to a statin, not to nothing [1, 2, 3, 4, 5, 6, 11, 14, 15]. That is the wrong question. The question is: if you never take a statin, what else works? The answer the studies give is: we do not know, because nobody ran that trial. The one trial that did compare a non-statin to placebo, the Coronary Drug Project with niacin, showed a reduction in nonfatal heart attacks but no mortality benefit during the 6-year trial, only a delayed effect seen 15 years later in a post-hoc analysis [27].

The data that exists for the alternatives is a collection of 4-to-52-week lab value studies. Ezetimibe added to a statin drops LDL another 15-20%, PCSK9 inhibitors drop it around 60%, and the combination of fenofibrate and ezetimibe shifts LDL particle size slightly larger [1, 2, 5, 6, 10, 11, 13]. But lowering a surrogate endpoint is not the same as preventing a heart attack. The IMPROVE-IT trial (not in this evidence set) is the only major trial that showed ezetimibe plus statin beats statin alone for hard outcomes, and the absolute benefit was tiny: about 2 fewer events per 100 patients over 7 years.

Treatment vs. nothingLDL reductionTested for death or heart attack vs. nothing?
Ezetimibe alone~18% [1]No
PCSK9 inhibitor alone~60% [10]No
Niacin alone~15%Yes (CDP: no mortality benefit in 6 years) [27]
Fenofibrate alone~10%No
Bile-acid sequestrant alone~15-20%No

My call: if you refuse statins, the evidence for any alternative preventing a heart attack or death is absent for ezetimibe and the PCSK9 drugs, and weak at best for niacin. You are trading a proven-but-risky drug for drugs whose benefit is measured on a lab test, not in outcomes. Confidence: high that the evidence does not support the claim these drugs improve survival in people who do not take statins.
