The short version: IL-6 triggers atrial fibrillation through at least three well-documented pathways, but the evidence comes from animal models and human tissue studies, not from vaccine-injured patients.

The 2025 review synthesizes the mechanism clearly. IL-6 is a downstream product of NLRP3 inflammasome activation, and that inflammasome drives atrial electrical remodeling, fibrosis, and calcium mishandling [11]. In human atrial tissue from 46 AF patients, there was significantly more immune cell infiltration than in sinus rhythm controls [11]. In diabetic and obese patients, epicardial fat tissue pumped out IL-6, TNF-α, and IL-1β, creating a pro-inflammatory milieu that directly primes the atria for fibrillation [11]. Animal models confirm the causal chain: NLRP3 knockout mice were protected from atrial remodeling, and blocking the inflammasome with MCC950 or glibenclamide reduced AF inducibility [11].

The second pathway is sympathetic overdrive. IL-6 activates the sympathetic nervous system, and renal sympathetic denervation in a canine model suppressed AF induced by atrial ischemia [19]. Norepinephrine levels in the right atrium reached 242 ng/mL in that model, showing the local sympathetic storm that IL-6 can trigger [19].

The third pathway is structural. IL-6 promotes atrial fibrosis through the NLRP3-caspase-1-galectin-3 axis, and in diabetes models that fibrosis directly increased AF susceptibility [11]. Fibrotic tissue conducts electricity poorly, creating the re-entry circuits that sustain AF.
