What is inside a vitamin b12 injection

A standard vitamin B12 injection contains the active ingredient, which is one of several forms of cobalamin. The most common is cyanocobalamin, a synthetic form where a cyanide molecule is attached to the cobalt atom at the center of the vitamin. Hydroxocobalamin is another form, sometimes used for specific metabolic disorders [2]. The injection also contains water and a preservative, usually benzyl alcohol, to keep it sterile in multi-dose vials. Sodium chloride or other salts are added to match the body's natural salt concentration, so the injection does not sting as much. Some formulations include a buffer like sodium hydroxide or hydrochloric acid to adjust the pH to a neutral range, around 5.0 to 7.0, which is less irritating to muscle tissue.

The evidence we retrieved does not include a single manufacturer's package insert or a regulatory filing that lists the exact excipients for a standard B12 injection sold in the United States. The studies we have are about other things: one tests a buccal film as an alternative to injections [1], another uses B12 injections in a hepatitis outbreak investigation [20], and a case report describes an allergic reaction to cobalt from B12 shots [14]. None of them list the full ingredient list of a commercial B12 vial. That absence is itself worth noting. The formulation information that exists in the literature is scattered across studies of related drugs, not B12 itself. For example, a review of monoclonal antibody formulations lists common injectable excipients like polysorbate 80, mannitol, and histidine buffers [7], and a study of docetaxel shows that polysorbate 80 can trigger complement activation in dogs [6]. But these are not B12 injections. The Cochrane review comparing oral to injected B12 does not list the injection's ingredients either [15].

My call: the ingredients of a standard B12 injection are well-known general pharmaceutical knowledge (cobalamin, water, preservative, salt, pH buffer), but the specific formulation for any given brand is not in the evidence we have. Confidence: moderate for the general composition, low for the exact brand-specific recipe.
