Is ivermectin best administered to humans using tablets

The short version: oral ivermectin tablets are the standard, well-studied route for parasitic infections, but the evidence for COVID-19 is weak and the safety data is thin for off-label use.

The evidence you retrieved is almost entirely about oral ivermectin tablets for parasitic diseases like scabies, strongyloidiasis, and filariasis. For those conditions, the tablet is the established route. Studies show it works: a single oral dose of 200 µg/kg cured about 76% of strongyloidiasis cases compared to 38% for albendazole [25], and it outperformed topical malathion for head lice, with 95% of patients lice-free at day 15 versus 85% [9]. The pharmacokinetic data from 2003 and 2006 [1, 4] shows the oral route produces measurable blood levels, and the 2015 scabies trial found oral and topical ivermectin had similar cure rates after two weeks [8]. For these parasitic infections, the tablet is the best-studied and most practical route.

For COVID-19, the picture is different. A 2025 meta-analysis of 23 randomized trials with over 12,000 patients found no statistically significant improvement in viral clearance, hospitalization duration, or symptom resolution with ivermectin [24]. A pharmacokinetic modeling study from 2020 calculated that the standard oral dose of 200 µg/kg produces lung concentrations only about one-fifth of the level needed to inhibit SARS-CoV-2 in the lab [37]. The evidence for COVID-19 does not support the oral tablet as effective.

Safety is the other concern. A 2023 case report documented a 70-year-old woman who developed drug-induced liver injury after taking oral ivermectin 12 mg daily for COVID-19 prophylaxis, with a RUCAM score of 8 indicating a highly probable association [14]. A 2009 study found ivermectin inhibits several mammalian P-type ATPases at concentrations in the low micromolar range, including the sodium-potassium pump and SERCA, which is a mechanism for potential toxicity at higher doses [19]. The safety data for long-term or high-dose oral use is not robust.
