Is leaky gut syndrome real?

The short version: leaky gut is a measurable biological reality, not a diagnosis you can hang a treatment protocol on with confidence.

The evidence you retrieved shows that intestinal permeability can be measured with sugar probes like lactulose and mannitol, and that it is abnormal in a long list of conditions: Crohn’s disease, liver cirrhosis, celiac disease, autism spectrum disorder, irritable bowel syndrome, psoriasis, NAFLD, and even after acute kidney injury or circadian disruption [1, 4, 10, 14, 24, 25, 23, 16]. The 2019 Gut review by Camilleri et al. calls it a real phenomenon with established measurement methods and disease associations [11]. The 2016 Current Opinion review lists over a dozen diseases linked to barrier dysfunction [26]. So the concept is real.

The problem is that the evidence does not tell you what to do about it. The best interventional study here is the 2018 glutamine trial in postinfectious IBS-D, a randomized placebo-controlled trial where 5 g of glutamine three times daily normalized permeability and cut IBS symptom scores by 40% [19]. That is a real result in a defined population. But it is one trial, and glutamine is not a cure-all. The flavonoid-rich licorice extract study in rats and the probiotic studies in mice show barrier protection in animal models [28, 17, 22]. Human data on diet, probiotics, or other interventions for leaky gut outside celiac and IBS is thin to nonexistent.

My call: leaky gut is a real, measurable phenomenon with strong mechanistic plausibility and documented disease associations, but the evidence for any specific treatment outside glutamine for postinfectious IBS-D is not clear. Confidence: moderate that the phenomenon exists, low that any general intervention works.
