How long after drinking would liver enzymes typically normalize?
The short version: after you stop drinking, liver enzymes start improving within days and usually normalize in 2 to 6 weeks, but the exact timeline depends on which enzyme you track and how damaged the liver was.
The 2016 gentiopicroside rat study shows that chronic ethanol feeding for 16 weeks raised AST to 868 U/L and ALT to 301 U/L, and the treated group dropped AST to 181 and ALT to 189 over the same period [5]. That is a massive drop, but it happened while the rats were still getting ethanol plus the protective compound, not during abstinence alone. The human data is more useful. The 2009 study of 76 heavy drinkers without cirrhosis tracked adiponectin during withdrawal: it fell from 19.6 at week 0 to 14.1 at week 2, 11.8 at week 4, and 9.6 at week 6 [26]. That is a proxy for liver recovery, and it shows the first two weeks produce the steepest change.
The 2017 baclofen cohort of 107 alcohol-use-disorder patients with liver disease gives the most direct answer. Over 12 months of abstinence, GGT dropped by a median of 82 U/L, ALT by 10.5 U/L, and bilirubin by 4.5 mg/dL, all statistically significant [21]. The GGT drop is large because GGT is the most alcohol-specific marker and takes longer to fall. AST and ALT, which leak from damaged hepatocytes, clear faster — usually within days to a few weeks once the alcohol stops.

My call: if your AST and ALT are only mildly elevated (under 100), expect them to drop noticeably within two weeks of stopping and normalize by six weeks. If GGT is high, that will take longer. The evidence is moderate — the human studies are observational and the animal studies use doses far above human drinking patterns, but the physiology is consistent.
