How long can protracted withdrawal can go on for
The short version: protracted withdrawal from tramadol can last months, and the evidence on how long is thin.
The best human data comes from a 2021 cohort study of 83 tramadol-dependent men in Iran [9]. After detox and six months of abstinence, their cognitive function improved significantly but still did not fully match healthy controls. On the Trail Making Test, a measure of processing speed and executive function, patients improved from a mean of 58.8 seconds at detox to 30.3 seconds at six months, while controls scored 37.0 seconds at baseline [9]. Memory and IQ also improved but remained below control levels. This tells you that measurable brain changes persist at least six months after stopping.
Animal data shows even longer effects. A 1981 study of rats withdrawn from LAAM, a long-acting opioid, found abnormal EEG and behavioral responses to morphine challenges lasting up to three months, with full normalization only at six months [6]. A 2026 rat study found that tramadol and codeine caused dose- and time-dependent drops in BDNF, a protein critical for brain health, in both blood and hippocampus [8]. After 56 days of tramadol at 60 mg/kg, serum BDNF fell from about 55 ng/L to 24 ng/L, and hippocampal BDNF from about 114 ng/L to 51 ng/L [8]. BDNF is involved in mood, memory, and nerve repair. Low levels are linked to depression and cognitive decline. The longer the exposure, the deeper the drop.
A 1996 review of methadone detox reported that only 19% of patients completed detox, and among those who did, only 6% were abstinent at 6-12 months [5]. This is not tramadol-specific, but it shows that protracted withdrawal from long-acting opioids is a real barrier to recovery.
