How long does variant-matched protection last, especially in older adults?

The short version: the evidence retrieved does not answer how long variant-matched protection lasts in older adults, because it contains no study that follows matched versus unmatched vaccinated people over time on hard outcomes in that age group.

None of the 30 records retrieved compares variant-matched to ancestral vaccines in older adults with a primary outcome of hospitalization or death. The closest match is a narrative review [27] reporting that the GSK and Pfizer RSV vaccines for older adults show 82-89% efficacy against RSV-LRTD in the first season, dropping to 56-78% in the second season. That is a different virus and a different vaccine platform. Another study [29] gave hACE2 mice an ISA720-adjuvanted ancestral spike and then challenged them, reporting 40% survival in aged mice versus 100% in young. Mouse data does not extend to humans.

The evidence on mRNA COVID-19 boosters in older adults is limited to one hemodialysis study [11] that measured antibody titers, not infection or death, and compared vaccinated dialysis patients to age-matched vaccinated controls. No record here includes a true unvaccinated older-adult comparator group. No record follows a cohort past a few weeks for safety. The burden of proof has not been met for the question asked.

My call: I cannot tell you how long variant-matched protection lasts in older adults because the studies designed to answer that were not retrieved. The gap itself is the finding. Confidence: not clear.
