How long will a Parkinson’s patient live

Parkinson's is a progressive neurological disease with a highly variable course. Life expectancy depends far more on the subtype and age at onset than on the diagnosis itself.

The single most useful number comes from a 2020 study that broke Parkinson's into three phenotypes [8]. The mild motor-predominant form, which accounts for about half of all cases, has a mean survival of about 20 years from diagnosis. The intermediate form averages about 13 years. The diffuse malignant form, which involves early cognitive decline and postural instability, averages about 8 years. Only about 16% of patients have this aggressive form.

Age at diagnosis matters enormously. A 1989 study found that patients diagnosed in their 40s had a death rate about 1.8 times higher than expected for their age, while those diagnosed in their 60s had a rate about 2.2 times higher [1]. That sounds similar, but the younger group lived decades longer in absolute terms. A 2018 study confirmed that the postural instability gait difficulty (PIGD) subtype roughly doubles the risk of death compared to the tremor-dominant subtype [4]. The same study found that smoking, lower education, and higher depression scores all shortened survival.

These are averages. Individual variation is wide. The mild form has a standard deviation of nearly 8 years, meaning many patients live well past 25 years. The diffuse malignant form has a standard deviation of about 5 years, so some patients decline much faster.

The evidence here does not include any intervention trials that directly measure whether treatment extends life. The studies are observational, tracking people who received standard care. What they show is that the disease itself is not a single entity. A patient with tremor-dominant, young-onset disease has a fundamentally different trajectory from one with postural instability, cognitive decline, and older age. The question to ask your neurologist is not "how long do I have" but "which phenotype do I have."

My call: life expectancy ranges from about 8 years for the aggressive form to over 20 years for the mild form, with age at onset and motor subtype as the strongest predictors. Confidence: moderate. The phenotype data comes from a single large cohort and needs replication.
