Can low testosterone be safely treated, and does it improve erections?
The short version: testosterone therapy is a real treatment with real risks, and it helps erections modestly, mostly when the level is genuinely low.
For a man with confirmed low testosterone, therapy does improve erections, but not dramatically. In one review, 40 to 60% of hypogonadal men with erectile dysfunction improved on testosterone [2]. A 2006 study of 22 men found 12 had significant erectile improvement and 10 did not [8]. Testosterone's bigger effect is on desire, not erection capacity [2].
The honest catch: adding testosterone to a PDE5 inhibitor like sildenafil gave only a 2-point bump on the erectile function score, and that bump was not statistically significant [1]. A 2022 review found the same pattern, with placebo-controlled trials often non-significant [3].
| Finding | Result |
|---|---|
| Hypogonadal men improved on TRT | 40-60% [2] |
| TRT + sildenafil vs sildenafil alone | +2 points, not significant [1] |
| TRT + vardenafil in prior non-responders | IIEF-EF 12 to 24 [4] |
Safety is where the evidence gets thin and conflicted. One large veterans study found testosterone linked to higher heart attack, stroke, and death risk [7]. Other analyses found no excess cardiovascular risk in the T Trials [6], and a large trial (TRAVERSE) showed roughly equal cardiac events but more atrial fibrillation, kidney injury, and clots on testosterone [9]. Long-term safety data is limited [5].
My call: worth trying if your testosterone is genuinely low and symptoms fit, with monitoring of hematocrit, PSA, and heart risk, and honest acknowledgment that the erection benefit is modest and the long-term safety picture is unsettled. Confidence: moderate.

Hypogonadism and erectile dysfunction: the role for testosterone therapy
In hypogonadal men with erectile dysfunction, testosterone therapy may augment ED treatment, particularly when combined with PDE5 inhibitors; testosterone alone often does not reliably restore erectile function.
