Mansanguan study of Pfizer covid safety from 2022
The short version: the evidence confirms a real, dose-dependent risk of heart inflammation in young males after Pfizer's mRNA shot, but the long-term consequences are barely tracked.
The evidence here is a patchwork of passive surveillance and manufacturer-funded trials, not a single head-to-head trial of vaccinated versus unvaccinated people followed for years. The New Zealand nationwide study found a statistically significant increase in myo/pericarditis, with a risk difference of 3.1 per 100,000 after the second dose [2]. The BMJ living systematic review puts the incidence in male adolescents at 50-139 cases per million doses, and importantly, it found that over half of followed patients still had persistent echocardiogram abnormalities or ongoing symptoms at three months [19]. A Korean national surveillance of adolescents found 184 confirmed cases out of 8.1 million doses, with 89% classified as mild, but that still leaves 11% who were not mild [17]. The Thai study reported 10 deaths among 204 total cases of myocarditis and pericarditis across all vaccine types [18]. A case report documents a 22-year-old man who died from myocarditis confirmed on autopsy five days after his Pfizer shot [11]. The Puebla pediatric passive surveillance found 52 adverse events per 2 million doses, including 2 seizures and 1 case of acute flaccid paralysis [27].
The evidence does not include a single long-term, controlled study comparing all-cause mortality or serious illness in vaccinated versus unvaccinated young people. The safety data stops at a few months of follow-up for most outcomes. The claim that the vaccine is "safe and effective" for this population rests on short-term antibody data and passive reporting systems that miss most injuries by design.

My call: the evidence shows a clear, dose-dependent risk of heart inflammation in young males that is not trivial, and the long-term consequences of that injury are not known because nobody has done the study. Confidence: moderate for the acute myocarditis risk, low for long-term safety.
