Is MARPE (mini screw expander) worth the risk for OSA ?

MARPE can widen the upper jaw and improve nasal airflow, but the evidence that it meaningfully treats obstructive sleep apnea (OSA) in adults is thin and indirect.

The retrieved evidence shows MARPE reliably expands the maxilla. A case report on a 22-year-old woman with mild OSA (AHI 7.9) saw her AHI drop to 1.5 after 8.8 mm of expansion [2]. That is a single patient, not a trial. A systematic review found MARPE improved nasal airflow and resistance in children and adolescents, but did not find significant changes in airway volume [1]. The same review reported that one study found a statistically significant improvement in nasal airflow (mean difference 52.7 cm³/s, p=0.020) and a reduction in nasal resistance [1]. These are surrogate endpoints—better airflow does not automatically mean fewer apneas.

The strongest airway data comes from surgically assisted rapid maxillary expansion (SARPE), not MARPE. A prospective study of 18 adults who underwent SARPE found the lower pharyngeal airway area increased by about 54% (p=0.0016) [3]. That is a large structural change, but SARPE is a major surgery with its own risks. MARPE is designed to avoid surgery, yet no study in this retrieval directly compared MARPE to a sham or untreated control for OSA outcomes in adults.

The risks of MARPE include screw loosening, infection, pain, and relapse. A meta-analysis of surgically assisted expansion found that intercanine width relapsed by about 1.5 mm (p=0.02) over the long term [4]. MARPE likely has similar or greater relapse, but long-term stability data is sparse. The evidence base is funded by academic institutions, not manufacturers, but it consists mostly of small observational studies and case reports—no large randomized trials with hard OSA endpoints.

My call: MARPE may help a narrow subset of adults with mild OSA and clear maxillary constriction, but the evidence does not support it as a general OSA treatment. Confidence: low.
