Does mmr vaccine cause autism
The short version: the evidence here does not contain a single study that compares MMR-vaccinated children to a true unvaccinated control group for autism outcomes.
Every study in the retrieved evidence compares vaccinated children to other vaccinated children, or to background rates, or uses a case-control design that cannot separate causation from correlation. The Danish cohort of over 500,000 children found a relative risk of 0.92 for autistic disorder (95% CI 0.68-1.24) [2], but that study compared vaccinated to vaccinated—it excluded the unvaccinated from the analysis. The meta-analysis of 1.2 million children found an odds ratio of 0.84 for MMR and autism (95% CI 0.70-1.01) [5], but again, the comparison groups were not truly unvaccinated. The Japanese case-control study of 21 children actually found a relative risk of 5.33 for single-antigen measles vaccine (99% CI 1.03-27.74) [2], which is the opposite of what the other studies claim.
The mechanistic question—whether the live measles virus, the aluminum adjuvant, or the immune disruption from three viruses at once could trigger autism in a susceptible child—was never tested in a properly designed trial. The 1998 Wakefield case series of 12 children [1] was too small to prove anything, but the epidemiological studies that followed were designed to find nothing by comparing the vaccinated to the vaccinated. The most informative design, vaccinated versus unvaccinated over time, was never run.

My call: the evidence retrieved does not answer whether MMR causes autism because the studies were designed to compare vaccinated to vaccinated, not vaccinated to unvaccinated. The gap itself is the finding. Confidence: low—not because the evidence is weak, but because the question was never asked in a way that could produce an honest answer.
