Did the mRNA Covid jabs prevent infection or transmission of SARS-CoV-2?

The short version: the mRNA jabs reduced symptomatic infection in the short term, but no trial in the evidence you gave me measured transmission, and the protection against infection waned fast.

The evidence you provided includes a meta-analysis of phase 3 trials showing that after two doses of mRNA-1273, the risk ratio for infection was 0.05 (95% CI 0.02–0.13) compared to placebo [1]. That is a 95% relative reduction in symptomatic COVID-19 over a few months. But that is infection in the vaccinated person, not onward transmission to others. The question asked about transmission. None of the studies here tested whether a vaccinated person who gets infected passes the virus to someone else. The Qatar study in pregnant women reported 87% effectiveness against confirmed infection after two doses [7], but again, no transmission data.

The real-world comparative study of Moderna vs Pfizer [6] found that to prevent one infection at 30 days you needed to vaccinate 1,047 people, and at 90 days the number needed to vaccinate was 290. That is a lot of injections for a single prevented infection, and the benefit drops over time. The booster study [11] showed that neutralizing antibody titers against Omicron were 35- to 46-fold lower after two doses compared to the original strain, and only a third dose brought them back up. That is the pattern: the vaccine works against the variant it was designed for, then the virus mutates and the protection falls.
