How much fat would be allowed in a Mediterranean diet
The Mediterranean diet does not set a maximum fat percentage. It sets a fat quality standard: replace saturated and industrial fats with olive oil, nuts, and fish.
The evidence is consistent on what matters. The Seven Countries Study found that the ratio of monounsaturated to saturated fat (MUFA/SFA) predicted coronary heart disease mortality better than total fat intake did [4]. A meta-analysis of 32 cohort studies found that higher MUFA intake, especially from olive oil, was linked to lower all-cause and cardiovascular mortality [3]. The PREDIMED trial tested a diet where fat provided about 40% of calories from olive oil and nuts, and it cut heart attacks and strokes by about 30% compared to a low-fat control [1][5]. That is not a low-fat diet. It is a high-fat diet built from the right sources.
What the numbers actually look like comes from a 2025 cross-sectional study of 1,342 adults. Those with high Mediterranean diet adherence ate about 32% of calories as fat, with 13% as MUFA and 9% as SFA [2]. Those with low adherence ate 28% fat, with only 6% as MUFA [2]. The high-adherence group had lower body fat, lower inflammation (hs-CRP 1.26 vs 4.03 mg/L), and more muscle [2]. The difference was not total fat. It was fat quality.

The practical answer: there is no upper limit on fat from olive oil, nuts, and fish. The limit is on animal fats and industrial oils. A Mediterranean diet can be 35–40% fat and still be protective, as long as the fat is mostly monounsaturated and polyunsaturated from whole foods [1][2][3]. The question is not "how much" but "from where."
