How much does the HPV vaccine reduce warts or precancer in men?
The short version: the evidence shows HPV vaccine reduces genital warts and precancer in men, but the numbers come from modeling and women-only trials, not from a direct trial in men.
The evidence you retrieved does not contain a single randomized trial testing HPV vaccine against genital warts or precancer in men. The 2007 review [2] reports 100% efficacy against HPV-related disease, but that result comes from over 20,000 women, not men. The modeling studies [4, 5] estimate herd effects in Australia using female-only vaccination programs, then project what male vaccination would add. The Taiwan study [3] shows men with genital warts have a 5.87-fold higher risk of anogenital cancer, but that is a risk marker, not a vaccine trial.
What the evidence does show is that HPV types 6 and 11 cause roughly 90% of genital warts, and types 16 and 18 cause most HPV-related cancers in both sexes [1, 5]. The quadrivalent vaccine targets those four types. In women, it reduced vaccine-type HPV prevalence from 28.7% to 6.5% after the program started [5]. In Brazilian women, vaccine effectiveness against any HPV infection was 64% [7]. But these are surrogate endpoints—infection, not disease—and none of the studies compared vaccinated men to unvaccinated men for the outcomes you care about.
