Muscle bracing is a voluntary co-contraction of the abdominal wall and lower back muscles to stiffen the torso. It is not a drug or a vaccine, so the burden-of-proof question is about whether the evidence supports its use for injury prevention or pain reduction.

The evidence here is thin and mostly comes from small, short-term studies on core stability exercises, not from long-term trials on injury rates. One ten-year follow-up of 600 people with recurrent low back pain found that exercise frequency mattered more than exercise type, and that adding abdominal bracing to either strengthening or flexibility exercises improved outcomes [4]. That is the strongest piece of evidence in this set. A 2014 study in middle-aged women found that bracing exercises increased the cross-sectional area of the external oblique, internal oblique, and transversus abdominis muscles more than hollowing exercises did [3]. What the evidence does not show: whether muscle bracing prevents first-time injury in healthy people, whether it reduces re-injury rates after recovery, or whether it has any effect on long-term joint health. The studies here are all about short-term muscle activation or pain in people who already have back pain. No study in this set compared bracing to no bracing for injury prevention in athletes or general populations. The mechanism is plausible—stiffening the torso reduces excessive motion that could strain ligaments or discs—but the evidence for real-world injury reduction is absent from these records.

My call: muscle bracing is a reasonable technique for people with recurrent low back pain who are already doing supervised exercise, but there is no evidence from these studies that it prevents first-time injury or that it works better than simply doing any consistent exercise program. Confidence: low for injury prevention, moderate for short-term muscle activation in people with back pain.
