My apple watch has told me my walking heart rate has dropped by 15 points for the last 17 days. Is this serious?
A 15-point drop in walking heart rate for 17 days is worth checking, but it is not automatically an emergency.
That is a real change, not noise. Normal resting heart rate for adults is often quoted as 60 to 100, but one large study of 500 normal people suggested the real range is closer to 50 to 90 [5]. A sustained drop of 15 beats per minute could simply mean you are getting fitter. Endurance training reliably lowers resting heart rate, and studies in athletes show this comes from increased parasympathetic tone, the "rest and digest" branch of the nervous system [1, 4]. If you started exercising more, lost weight, or improved your sleep in the last few weeks, that alone could explain it.
But it can also signal something real. A slow heart rate can come from thyroid problems, electrolyte shifts, or sick sinus syndrome, where the heart's natural pacemaker falters [11]. Certain medications, including beta-blockers, directly lower heart rate [19, 26]. Even some steroids given intravenously have caused significant bradycardia in children [10]. If you take any new medication, that is the first suspect.
The watch itself is the other variable. Wrist-based optical sensors are not perfect. One study found that only about half of the data they collect is usable for cardiovascular analysis, and quality is much better at night than during the day [33]. A change in how the watch sits on your wrist, or a software update, can shift readings without your body changing at all.
What matters most is how you feel. Dizziness, fainting, chest pain, unusual fatigue, or shortness of breath with that slow heart rate means you should be seen promptly. If you feel fine and the drop is steady, it is reasonable to check in with a doctor and get a simple ECG, but you do not need to panic.
